Passport application has to be filled online at the following link: https://mportal.passportindia.gov.in/mission/
Eligibility:
- An Indian Passport holder is eligible to obtain a new passport when his/her current Indian Passport expires.
- The earliest an Indian national can apply for a new ten-year Indian Passport is a year before the expiry of his/her present passport.
- The fresh Indian Passport that is provided to the applicant has ten year validity. To put it clearly, if an Indian Passport holder has his/her passport expiring on December 31, 2023, application for a new Indian Passport can be submitted with the Embassy on any working day (as per the timings) between January 1, 2023 and December 31, 2023.
- Please note that Government of India has stopped the practice of issuing Additional Booklets on existing Indian Passports. The practice of adding pages to existing passports has also been done away with.
- Hence, in case of either the pages of the existing passport getting exhausted or in case of completion of validity of the passport, the applicant is required to apply for a new Indian Passport only, as per the required process.

Process:
- Applicants please note that Indian Machine Readable Passports (MRP) are not prepared in the Embassy and issued to the individuals from its office in Bratislava.
- Passports are prepared and printed by the authorities in India and sent to us for onward dispatch to their rightful owners.
- The process starts with the applicant depositing completely filled in Passport Application along with all the necessary documents; the case is then referred to authorities in India.
- If everything is found correct by them, the passport is printed and sent to the Embassy.
- Applicants have to be careful about giving proper information in the application form, depositing necessary documents including photocopies wherever necessary and provide appropriate colour photographs (ICAO guidelines to be followed).
- Photos that do not meet the prescribed specifications (ICAO guidelines) will not be accepted and application process will not be initiated.
- All the personal details should be filled correctly. Incorrectly or partially filled forms will not be accepted.
- Your Indian permanent address should be filled in the field of address to be printed and your local permanent address should be filled in the field of other address.
- Registration of birth of a minor child should be done at an Indian Embassy under Section 4(1) of the Citizenship Act, 1955.
- In case of minors, the form should be signed by both the parents and their passport copies should be attached with the process.
- All individuals seeking to apply for a new passport are required to deposit the application form and documents in person at the Embassy.
- One photograph to be pasted on the first page in the given space.
Processing Time:
- All passports are now being printed in India and therefore it will take about THIRTY DAYS for issue of a printed passport, i.e. if everything is found to be in order in respect of the application form, photo, etc., by the authorities in India.
- In case a reference is to be made to the original Passport Issuing Authority, it might take longer time. Applicants are advised to contact the Embassy 30 days after submitting their application forms to check the status of their new passport.
General Requirements for Passport Application (for all type of Passport application circumstances as mentioned below):
- Application form completely filled in online at https://mportal.passportindia.gov.in/mission/
- One recent colour photographs front profile with white background (5 x 5cm) as per ICAO guidelines.
- Original passport and copy of relevant pages.
- Copy of Slovak Residence Permit (original to be produced for verification).
- An Indian address with contact number (Mobile/Landline number) should be filled in the application form indicating either in Passport printable address details or other address details
Reissue of passport after the expiry of validity or within one year of final validity:
- Fee Euro 60.00 and Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.01.2013
- In case of Jumbo Passport Fee Euro 75.00 and Consular surcharge of Euro 2.00 and a written request application letter for reason for applying Jumbo passport.
Issue of passport in lieu of lost/stolen/damaged passport: If an Indian national who is legally resident in Slovakia has either lost, damaged his/her Indian Passport or the passport has been stolen, he/she is required to approach the Embassy of India, Bratislava to get a passport reissued. In case an individual on a short visit to Slovakia loses his/her passport wants to depart for India urgently, an Emergency Certificate is issued to him/her. Emergency Certificate issued to an Indian national by the Embassy does not validate travel to any other country and is meant for one way travel to India only. Documents required for issue of passport in lieu of lost/stolen/damaged passport:
- Original Police Complaint Report in case of loss or theft alongwith English translation.
- A statement by the applicant explaining circumstances in which passport was lost/stolen/damaged.
- Fee Euro 110.00 and Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.01.2013
Addition of spouse name in the passport:
- Copy of Marriage certificate (Original to be produced for verification). Marriage certificate issued in India should have the apostille/attestation for Ministry of External Affairs, India. If issued in Slovakia, apostille stamp for the relevant authority alongwith English translation from authorised translator.
- Fee Euro 60.00 and Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.01.2013
Deletion of spouse name from the passport:
- Copy of Divorce certificate (Original to be produced for verification). Divorce certificate issued in India should have the apostille/attestation for Ministry of External Affairs, India. If issued in Slovakia, apostille stamp for the relevant authority alongwith English translation from authorised translator.
- Fee Euro 60.00 and Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.01.2013
Issue of passport in case of change of address: If an Indian national wishes to change his/her address in the passport, the following additional documents are required to be submitted:
- Documentary evidence showing new address like Aadhar Card, Voter ID card, electricity bill etc.
- Fee Euro 60.00 and Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.01.2013
Issue of passport in case of change of appearance: If the appearance of an applicant has changed significantly since the issue of his/her last passport, an Indian national can apply for a fresh passport with recent photograph. The following additional documents are required to be submitted:
- A notarized affidavit explaining the circumstances under which the service is required.

Issue of passport in case of change of name (surname/middle name) following a marriage of Indian women in their passports: Upon marriage, if Indian women citizens wish to change her name (change/addition of surname), she can do so by following a procedure of submitting the following additional documents with the Embassy:
Affidavit regarding change of name.
- Marriage certificate issued by Registrar of Marriages (it must be attested by the Home Department of concerned State Government and then attested/apostilled by the Consular Division in the Ministry of External Affairs in New Delhi).
- Fee Euro 60.00 and Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.01.2013
KINDLY NOTE THAT CHANGE OF NAME AFTER MARRIAGE AS PER THE ABOVEMENTIONED PROCEDURE IS ALLOWED TO A FEMALE INDIAN APPLICANT ONLY. TO CHANGE THEIR NAMES, INDIAN MALE APPLICANTS IN ALL CASES AND INDIAN FEMALE APPLICANTS IN CASES OTHER THAN MARRIAGE ARE REQUIRED TO FOLLOW THE NORMAL PROCEDURE AS MENTIONED BELOW:
Issue of passport in case of change of name (normal procedure for both male and female):
- Affidavit regarding change of name in prescribed format signed in the presence of Consular Officer.
- Original newspaper advertisement announcing change of name, published in two leading daily newspapers. One newspaper should be of area of applicant’s permanent address in India (one in English and the other in Hindi/local language) and the other of the area of his present address in Slovakia. Waiting time of 30 days is mandatory after the last publication.
- Fee Euro 60.00 • Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.01.2013
Issue/ re-issue of passport in respect of Minors in Slovakia: The requirements are given below:
- Birth certificate of child issued by Slovak authorities with Apostille Stamp and English translation.
- Passport of parents (copy & original).
- Copy of Slovak Residence Permit of parents (original to be produced for verification).
- Fee Euro 36.00 (in case of Minors upto 8 years old)/ Euro 40.00 (in case of Minors above 8 years) and Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.01.2013.
